1Which insurances do you accept?
We accept Providence, Pacific Source, United Healthcare, Blue Cross Blue Shield "Regence", Cigna, and Moda.
2What is your policy on "No-Shows" and "Same Day Cancelations"?
A fee equal to 50% of billing will incur if you fail to show up to your first appointment or if you cancel within 48 hours of your appointment. If you fail to show up for a second time the fee will be 100% of billing for that appointment. Fees must be paid prior to rescheduling. If you feel you will not be able to make it to your scheduled appointment please reach out to our office as soon as possible.
3How early should i arrive to my appointment?
If you have completed the intake forms that were sent to you after scheduling your appointment then you should arrive 5 minutes early. If you have not completed the forms then please arrive 15 min early.
4What forms of payment do you accept?
Beyond your health insurance coverage, we accept any US-issued and most internationally-issued magstripe or chip cards bearing a Visa, Mastercard, American Express, Discover, JCB, or UnionPay International logo. Our terminal also supports mobile wallets such as Apple Pay, Google Pay, and Samsung Pay.
5What is your policy on refills?
Please contact your pharmacy and find out if you have a refill available. If you do not, please have them send us a refill request. You may also request a refill via our texting service or the patient portal. Some medications require monitoring by exam and/or labs and therefore will not be filled without proper follow-up. Most refill requests can be completed within 2-3 days if you are stable on the prescribed dosage.
